What We Do

One team. From hospital to home.

Post-Discharge Care

After a hospital stay, our model is designed so a nurse practitioner visits within 48 hours — checking medications, coordinating with your doctors, and making sure nothing falls through the cracks.

Home Health

When skilled nursing or therapy is needed, our team will provide it — in your home, coordinated with your existing care team.

Today, 80% of Medicare patients never see a doctor after leaving the hospital. Half go home on incorrect medications. The 30-day readmission rate is 15.5%, and hospitals pay millions in federal penalties. The gap between hospital discharge and home recovery is one of the largest unsolved problems in American healthcare.

What We Stand For

Two commitments.

Accountable

When we say we'll do something, we do it. And we own the problem until it's solved, even if it didn't start with us. We don't say "that's not our department." If a patient's referral went nowhere, the specialist didn't call back, or a family can't make sense of a discharge summary — we don't point at the door, we walk them through it. We make the call, we do the introduction, we follow up. No orphaned patients, no task that gets lost between handoffs. We treat every patient like they're family — and that shows up in the small things: honest communication when we don't know, proactive outreach when we're worried, and a warm handoff every time we hit the edge of our scope.

Advocate

We make sure every patient has a team of highly educated, highly motivated people in their corner. Because with Homeplace Health, now they do. Whether a patient has family nurses coaching them or lives alone and sees a doctor once a year, they get the same quality of advocacy. We present the full spectrum of options — even when some are hard to access, expensive, or off the standard path — because an informed patient makes better decisions, and better decisions lead to better outcomes. We'll push back on a payer denial, flag a concerning symptom pattern, or question a recommendation when the data supports it. That creates friction. We accept the cost because the alternative is a patient who doesn't know what they don't know.

For Agency Owners

Your life's work deserves the right partner.

We're actively seeking home health and home care agencies across New England.

Physician-led, not corporate-driven.

We're two physicians, not a private equity fund. No corporate playbook. No reorg cycles. Your patients get a doctor in their corner — something no other buyer offers.

We keep it local.

We're building in New England, not rolling up agencies to flip in five years. Your staff stays. Your relationships stay. Your community keeps the care team they know.

We invest in your people.

Our technology handles the paperwork so your caregivers can focus on patients. We bring clinical resources — physician oversight, specialist coordination, care management — that small agencies can't afford alone.

Your voice matters.

We don't impose a corporate identity. We partner with founders who want to stay involved and owners who want to know their legacy is in good hands.

Why We're Building This

A letter from the founders.

When someone leaves the hospital, the system basically says "good luck." Eighty percent of Medicare patients never see a doctor after discharge. Half go home on the wrong medications. We think that's unacceptable.

We're two physicians who've spent our careers inside the healthcare system — at CMS, at major health systems, at technology companies building the tools that are supposed to fix these problems. We've seen what works and what doesn't. The clinical model works. A physician-led team that visits patients at home within 48 hours of discharge — reconciling medications, coordinating care, making sure no one falls through the cracks. What fails is the execution: corporate bureaucracy, reorg cycles, distant operators who've never met a patient.

Homeplace Health is our answer. We're building a physician-led practice, starting in communities where the need is greatest — where the nearest specialist is months away, where PCPs can't see you for six months, where the aging population is growing faster than the care infrastructure can keep up. We believe healthcare should come to you. And we believe the people delivering that care should be accountable for the outcome — not just the visit.

That's Homeplace Health.
