Why We're Building This
A letter from the founders.
When someone leaves the hospital, the system basically says "good luck." Eighty percent of Medicare patients never see a doctor after discharge. Half go home on the wrong medications. We think that's unacceptable.
We're two physicians who've spent our careers inside the healthcare system — at CMS, at major health systems, at technology companies building the tools that are supposed to fix these problems. We've seen what works and what doesn't. The clinical model works. A physician-led team that visits patients at home within 48 hours of discharge — reconciling medications, coordinating care, making sure no one falls through the cracks. What fails is the execution: corporate bureaucracy, reorg cycles, distant operators who've never met a patient.
Homeplace Health is our answer. We're building a physician-led practice, starting in communities where the need is greatest — where the nearest specialist is months away, where PCPs can't see you for six months, where the aging population is growing faster than the care infrastructure can keep up. We believe healthcare should come to you. And we believe the people delivering that care should be accountable for the outcome — not just the visit.
